Patients in low-income communities often have the greatest health care needs. When the health workers who serve them lack proper resources, it results in negative health outcomes which creates a cycle of increasing burden on providers and patients. Sostento interventions break this negative cycle with uplifting impact.
We believe that no one should fall through the cracks of our healthcare system, and our collaborative work with marginalized populations and the healthcare providers who serve them help us drive health equity and build healthier communities.
Our efforts increase access to lifesaving care and improve equitable health outcomes.
Eliminating transportation barriers to care for patients through Ubers coordinated by community health workers.
Understanding patients' needs and referring them to services and other resources through digital screening tools.
Building capacity for telehealth deployment in communities that lack local specialist providers.
Our Success Stories
"Being able to use Uber has truly brought peace of mind to those who have struggled with this barrier as they know their healthcare does not have to be put on hold due to transportation issues. They are incredibly thankful to be able to utilize this service at no cost to them! Thank you Uber and Sostento for allowing our clinic to serve those who may not always have access to transportation."
“HelplineSOS gave us the capacity to suddenly take on a lot more call volume than we ever have before, and it wasn’t just our patients calling in to get information about testing, scheduling, and vaccinations – it was our entire community. The ability to consistently respond month after month enabled us to show up for our community. We hope that the trust we have built during this process of being a place that stays open and responds is something that people will continue to rely on for other health needs in the years to come."
"By the end of April 2021, my organization was considering the discontinuation of our COVID vaccine program after the rush for the vaccine subsided. Volunteer pharmacists helped administer our first 600 doses during the immediate rush for the vaccine, but volunteers were less interested in providing assistance to combat vaccine hesitancy and administered fewer doses per day. The decision to continue providing the vaccine on-site was made because of the resources promised by Project Finish Line to help fund staff to continue these efforts."
